The Arrangement Process

During the arrangement process, we will assist you with the following

  • Obtaining death certificates
  • Selecting a casket, urn and/or grave marker
  • Arrangements for the funeral, memorial and/or burial service, or cremation
  • Preparing the obituary and submitting it to the newspaper(s) of your choice
  • Provide you with information about notifying the deceased’s employer, attorney, insurance company and banks
  • Offer grief support or direct you to other resources

Complete our on-line Arrangement Form

  • If you are reading this information because the death of your loved one is imminent, you may want to complete pages 1 and 2 of our on-line Arrangement Form to ensure that you gather all the information we will need to obtain the death certificate.
  • If your loved one has already passed, our Arrangement Form will guide you through collecting important information our Funeral Director will need when you meet with him.

Hospital Release Forms

  • If your loved one died while in a hospital, they will have you sign a form authorizing them to release your loved one to Heritage Oaks Memorial Chapel. 
  • You will also need to sign an additional release form that we will provide to you which we will take with us to the hospital.
  • If your loved one is at an Assisted Living, Health Care Center, or a Residential Care Home, you will provide documentation designating Heritage Oaks Memorial Chapel as the mortuary authorized to remove your loved one from their care.

Call us at (916) 791-2273 to set an appointment to meet with one of our Funeral Directors.

  • When you arrive, you will be greeted by your Funeral Director who will escort you to our Arrangement Room.
  • If your loved one has a pre-arrangement in place with us, we will review their file and gather any additional information that is needed.
  • If you have completed our on-line Arrangement Form, your Funeral Director will review the information to ensure that we have all the information needed to order Death Certificates, file with the Social Security Department, and file for a Burial Permit.
  • Your Funeral Director will ask you questions about your preferences for burial or cremation, a funeral or memorial service, the type of casket or urn you want to purchase, and the location of their final resting place.
  • If your loved one served in the US Armed Forces at any time, they are eligible for benefits. We are prepared to answer your questions and make arrangements for them to be placed at a National Cemetery.
  • We will provide you with information that will help you write your loved one’s obituary and plan a funeral or memorial service to honor them.
  • We will offer grief support or direct you to other helpful grief resources.
